How we use and protect your child's health information.

This notice describes how protected health information ("PHI") about your child may be used and disclosed by Calm Wellness Center, and how you can access this information. Please review it carefully.
What is PHI?
PHI is information we collect within the clinical relationship that identifies your child and relates to their past, present, or future health, healthcare, or payment for healthcare. This includes intake notes, lab results, treatment plans, billing records, and clinical communications.
How we use PHI
We use your child's PHI for treatment (providing care), payment (collecting fees), and healthcare operations (running the practice, quality review, training, scheduling). We store PHI in OptiMantra, a HIPAA-compliant practice management platform, with appropriate technical and administrative safeguards.
When we share PHI
With your written consent, we may share PHI with your child's other healthcare providers (for example, your pediatrician). We will not share PHI for marketing without your express written authorization. We may disclose PHI without your authorization in narrow legally required circumstances (suspected abuse or neglect, court orders, public health reporting, or to prevent imminent serious harm).
Your rights
- You have the right to inspect and obtain a copy of your child's PHI.
- You have the right to request corrections to your child's record.
- You have the right to an accounting of certain disclosures we have made.
- You have the right to request restrictions on uses or disclosures.
- You have the right to receive confidential communications via specific methods (mail vs. portal vs. phone).
- You have the right to a paper copy of this notice on request.
Marketing site privacy
Information you submit through this website (calmwellnesscenter.com) before establishing a clinical relationship is governed by our privacy policy, not by HIPAA.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with us at info@calmwellnesscenter.com or with the Secretary of the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
