Pediatric functional medicine concierge package · 4 months
For children who need a longer clinical container.
PANS/PANDAS. Chronic Lyme. Mold exposure. Autoimmune patterns. Autism-spectrum support. Multi-system chronic illness. The 4-Month Concierge Package gives families four months of structured care, testing, lab review, follow-up, and portal support.
Who this is for.
This package is for children whose story has too many moving pieces for a short plan: sudden-onset anxiety or OCD, repeated infections, suspected PANS/PANDAS, chronic Lyme or tick-borne exposure, mold history, autoimmune patterns, severe gut dysfunction, chronic pain, fatigue, eczema layered with gut symptoms, or multiple specialists who have each looked at one piece while the larger pattern stays scattered. It is also for parents who are overwhelmed by conflicting advice. You have supplements on the counter, lab results in a folder, notes from several specialists, and a child who still feels off. The work here builds the clinical picture, prioritizes what matters first, and adjusts as your child responds.
What is included.
- Initial comprehensive evaluation. The intake is 60 to 75 minutes depending on age and complexity. We take the whole history: symptoms, timeline, infections, medications, diet, sleep, stress, home environment, family patterns, prior labs, and what has already been tried.
- Lab review session. Standard and young-child packages include a dedicated 60-minute lab review visit when testing is complete.
- Three 30-minute follow-up sessions. Follow-ups cover ongoing assessment, nutrition, lifestyle coaching, stress reduction, habit change, implementation questions, and plan adjustments.
- GI Map functional stool testing when included. Standard and young-child packages include GI Map stool testing with an individualized report. Infant testing is considered separately.
- Comprehensive wellness plan. Food, supplements when appropriate, nervous-system support, environmental changes, sleep routines, referrals, and next clinical steps written in plain English.
- Portal support between visits. Active package families can ask implementation questions between sessions during regular business hours.
- Progress review and transition planning. By month 4, we discuss whether your child is ready for maintenance, needs an extension, should step down into membership-level support, or needs another clinician involved.
How the four months work.
Month 1: Build the map. We gather the story, review prior labs, identify the most likely drivers, and decide which testing belongs first. You leave with a first-step plan.
Month 2: Interpret and start. Results begin coming in. We explain what matters and how each finding fits your child’s actual symptoms. The plan becomes more specific.
Month 3: Adjust and measure. This is where many families need the most support. Some symptoms improve, some flare, and some patterns become clearer only after the first layer is addressed. We adjust the plan based on response and decide whether repeat labs make sense.
Month 4: Stabilize and decide. We look at what has changed, what still needs attention, and what level of support makes sense next. Some families graduate to maintenance. Some extend care. Some move into lower-touch membership support when available.
Why this is the right choice for complex cases.
Complex pediatric cases need enough time to see what is working. A single visit can be helpful, and these layered cases usually need a larger container for PANS/PANDAS flare patterns, mold exposure, chronic Lyme history, autoimmune clues, gut dysfunction, sleep disruption, school stress, and family overwhelm. The 4-Month Concierge Package gives the work that container:
- Time to build the full clinical picture.
- Testing chosen around the child.
- Enough follow-up to adjust when the first layer gets hard.
- Honest conversations when the plan needs a change or another specialist should be involved.
What’s included.
The package is structured by age and complexity, and the exact pricing is shared on your free consult so we can match it to your child’s situation:
- Standard 4-Month Concierge Package. A 75-minute initial evaluation, 60-minute lab review, three 30-minute follow-ups, GI Map stool testing with individualized report, portal messaging support, and a comprehensive wellness plan.
- Young child 4-Month Concierge Package. A 60-minute initial evaluation, 60-minute lab review, three 30-minute follow-ups, GI Map stool testing with individualized report, portal messaging support, and a comprehensive wellness plan.
- Infant 4-Month Concierge Package. A 60-minute initial evaluation, three 30-minute follow-ups, portal messaging support, and a comprehensive wellness plan. Testing is considered separately when appropriate.
HSA and FSA cards are accepted for eligible services. Superbills are provided for visits. Calm Wellness is a cash-pay practice; families pay the practice directly and may submit superbills for possible reimbursement. Additional labs, testing, and supplements may be recommended when clinically appropriate and are discussed before families order them.
Fit and boundaries.
- Urgent situations need urgent care. Immediate safety concern, suicidal thoughts, medical instability, or urgent infection treatment belongs with emergency care or the appropriate specialist immediately.
- Complex healing may take longer than four months. Four months is enough time to build the map, begin treatment, and see how your child responds. Some kids need longer.
- Recommendations come from the clinical picture. We base the plan on your child’s history, testing, tolerance, and what the family can realistically implement.
- Primary care stays involved. We work alongside pediatricians and specialists. With your consent, we are happy to share findings and plans.
Common fit questions.
Is this for PANS/PANDAS? Often, yes. PANS/PANDAS cases usually need a longer arc than GI Reset because infection history, immune activation, gut health, environmental triggers, psychiatric support, and school function can all be involved.
Is this for chronic Lyme or mold exposure? It can be, especially when the child has multi-system symptoms and needs a careful testing and support plan.
Can we do this by telehealth? In Pennsylvania and New York, telehealth is available when clinically appropriate. Families from other states can come to the Morgantown clinic for in-person care.
Do we have to start here? The free consult helps decide whether GI Reset, an initial intake, 4-Month Concierge care, or another referral makes more sense.
